MEDICAL FORMS
The Health Office is open from 8:15 a.m. to 4:00 p.m. Our health office are ready to assist and provide care to students when they are unwell or are injured during the school day. Health staff will contact the parents of a student regarding any health issues and concerns and supervise students who take prescription medication. Health office staff communicate with medical providers to coordinate medical care in a school setting.
By law, all prescription and non-prescription medication must have a signed physician form and signed consent from a parent
When a student needs to take medication during school hours, a medication form must be filled out by a physician and the parent/guardian. Forms are available on the school website or the Health Office. Medication(s) are administered by the Health Technician, LVN, or other designated school personnel, under the supervision of the School Nurse. It is the student's responsibility to come to the Health Office to take their medication. Students can self-carry and self-administer emergency medications (i.e., insulin, glucagons, Epi-pen, and inhalers) as ordered by a medical provider and with parent consent.
What to do when a student is ill and needs to go home during the school day.
Students are not to contact their parents before reporting to the Health Office. Health staff will assess, and provide care and intervention to students prior to calling parent/guardian. When the student needs to go home, staff will contact the parent/guardian and make arrangements to bring the student home. The parent/guardian may give verbal permission allowing for the student to walk home, a pass will be given student to leave school.
The student will be allowed to leave ONLY with permission from the people listed on the student's Emergency Card. It is imperative that you have an updated emergency card on file and you list a trusted relative or friend, in the event we are not able to reach you during a medical situation.
What if it's necessary for a student to leave campus for a medical appointment?
Parents need to excuse their students from the attendance office for medical appointments. Parent or guardian may also call the Attendance Office in advance for medical or dental appointments.
What happens if a student is kept in the Health Office for a class period or more?
When a student is in the Health Office for any length of time, they will be put excused by health staff during class period student was absent.
Important note: At the end of every school day, a list of the students who have been in the Health Office, who left the campus for an appointment, or who have gone home ill, will be given to the Attendance Office.
What happens if a student has a doctor's note not to participate in PE or has an injury that prevents normal mobility on campus?
All medical notes with request to exclude or limit students from physical activity in Physical Education class, or having an injury that requires assistance and accommodation, must be submitted to the health office. The health staff will notify teachers and staff accordingly. Heath staff may contact the doctor's office for any clarification of orders.
Mandated Hearing screening:
Hearing screening is mandated in Kindergarten/first grade and in second, fifth, eighth, tenth/eleventh, and upon first school entry (California Code of Regulations, Title 17, Section 2952 (c) (1) ). Hearing screening in California public schools must be conducted by a credentialed school audiometrist.
